A referral inquiry for Day 2 Day Services is an initial contact made by a person or organisation interested in utilising the services provided by Day 2 Day Services. This inquiry may come from an individual with a disability or their family member, a healthcare professional, or an NDIS planner, among others.
The purpose of the referral inquiry is to provide Day 2 Day Services with information about your specific needs and requirements as well as your goals and aspirations. This information allows Day 2 Day Services to better understand the your situation and determine how we can best support you.
During the referral inquiry process, Day 2 Day Services will ask a series of questions to gather important information, such as medical history, your support needs, and any specific goals you may have. This information will be used to determine the most appropriate services and supports to meet your individual needs.
We aim to respond to your enquiry within 5 business days. Your enquiry does not guarantee we can support you, it does however, enable us to ensure we have the right people to meet your needs.
We can include you on our waiting list should our services be at capacity.
